It was a joint vision. Even though we didn't see Ethan have the vision himself, we were told that it was a joint vision they both had thanks to Tabitha's green mist. Ethan even admitted it, so technically it's also Ethan's vision, so it counts.

Ethan never had any fantasies about Gwen. The only time I remember him ever including her in one of his fantasies, was when he had the Jane Eyre fantasy of Theresa and referred to Gwen as the 'beast in the attic."

Nope, they're all Ethan's. The clip with them as old people was a joint vision compliments Tabitha. Technically not a fantasy, but it was a 'greatest fears' kinda thing, so I thought it appropriate to add. Both their greatest fear was them never getting together.
